You will need a valid referral from your healthcare provider. Please also bring a list of your medications, past medical conditions, and any relevant imaging, such as ultrasounds. Additionally, include any correspondence related to your condition. Ensure you bring your Medicare card and/or private health insurance card. Finally, please bring a support person if you wish and a list of any questions you have for Dr Sarofim
